performance optimised bbcode module:

 - splitted module in three, so needed parts only loaded when required
   (this greatly helps PHP memory consumption)
 - replaced all underscores with hyphens in HTML class names
 - fine tuned multiline code handling
 - fixed help text to reflect newest install methods
